Individual Address Proof (Only required if Personal ID document does not contain address of individual) – Acceptable address proof includes bank statement, utility bill, current lease agreement, or resident permit.
- The address proof should be recent (less than 3 months old for bank statements and utility bills)
- Must show name and address
- Cannot be cropped or blurry
